Agreement Overview
This Purchase Agreement constitutes the entire agreement between EQ Pulse Wave, Inc. (“Seller”) and the Buyer and supersedes all prior communications.

Device Classification & Usage Disclaimer
EQ Flux Wave is a Class I FDA-registered therapeutic device intended for general wellness, performance recovery, and physiological support. It is not intended to diagnose, treat, or cure any medical condition.

Buyer agrees to:

  • Use the device according to provided safety guidelines
  • Ensure only trained personnel operate the device
  • Follow all contraindications

Contraindications include (but are not limited to):

  • Individuals with pacemakers or implanted electronic devices
  • Pregnancy
  • Active internal bleeding or hemorrhaging
  • Severe infections or unevaluated medical conditions

Limitation of Liability
EQ Pulse Wave, Inc. is not liable for injury, harm, or damage resulting from:

  • Use contrary to contraindications
  • Improper or unintended use
  • Unsupervised or non-professional operation
  • Device modification or tampering

Indemnification
Buyer agrees to indemnify and hold harmless EQ Pulse Wave, Inc., its officers, employees, and agents from any claims arising from misuse, negligence, or violation of these terms.

Return Policy

Any unopened and undamaged supplies can be returned within 2 business weeks if unopened and unused for a full refund to EQ Flux Wave within two weeks of receiving the order. If opened and used there will be a 50% return fee.

Customers shall be responsible for all return shipping fees. Any defective products are eligible for replacement within 30 business days of receiving the order. To begin the return process, please email info@eqfluxwave.com 

Inspection Requirement
Buyer must inspect the device within five (5) business days of receipt and report any damage or concerns. Failure to do so may void eligibility for any remedies.

Disclosure

If one or more of your items in your order has a ship date, your loan payment(s), including interest, may be due before the merchant ships all of the items. Note that you will not receive a rebate of any interest that you may have already accrued on an amount that later gets refunded.
